Preamble: Many of the issues I bring to you are very small, yet each issue is just that, an issue. If you take care of the small issues, then you will be able to build and more importantly, focus on the bigger issues...a larger practice and more family time.
In your SOAP notes, evaluation reports and narratives, never use only first names when referring to your patients. It is unprofessional and can also be construed as having an improper relationship when being expert for a patient. It can be a possible cause for a judge to recuse you from the case due to familiarity or having a personal relationship with the patient. A good defense lawyer can make that argument and often be successful. There is never an instance when you should refer to your patient in any document by first name only.
