“The Pitfalls of ALL in a Narrative”
In reading narrative after narrative over the last few years, there is a common pitfall that many fall into that is not good. That pitfall is the word “ALL.” As an example, "Mrs. Jones’ ranges of motion are all within normal limits or the muscles tested are all in spasm."
What ranges of motions? The entire ones you performed or every single one there is known to medicine and man? You need to be more specific. "The following ranges of motion were performed and found to be within the normal range: cervical flexion, extension, left and right lateral bending."
You have to be very specific and name the range, the muscle, the reflex, the nerve root, and the test. It all needs to be clearly defined where there is no question about what you did and what you found as a result of your clinical examination.
