Academy of Chiropractic Personal Injury & Primary Spine Care Program
Quickie Consult 130
“Take out the Six Shooter”
Here we go again…
It seems that many of you want to have a HUGE PI practice, but are not willing to do what it takes to be the best at what you do. The definition of insanity is to do the same thing again and again and expect different results.
A doctor who has been in the program for over 2 months sent me a narrative to critique after he purchased my template, thinking that was all he needed to do. The following are excerpts from my critique of his narrative.
I guess you didn’t read the consultations…or didn’t understand them…This is a “take the gun out and shoot yourself in the foot diagnosis.” A monumental mistake!!!! Didn’t you read the consultations?!?!?!
- 847.0 Cervical strain/sprain
- 847.1 Thoracic strain/sprain
- 847.2 Lumbar strain/sprain
- 729.1 Myofascitis, Cervical, thoracic and lumbar
- 739.1 Segmental dysfunction, Cervical
- 739.2 Segmental dysfunction, Thoracic
- 739.3 Segmental dysfunction, Lumbar
- 784.0 Headache
This entire diagnosis says nothing about bodily injury!!!!!!
At this time she states that her symptoms have subsided over the past 2 weeks. Objectively, all testing has improved substantially Did they only improve or did they resolve??? Real doctors use language like, “Foraminal compression was negative,” not, “Most tests improved…” with the exception of mildly reduced and painful cervical flexion and tenderness to palpation with mild muscle spasm and guarding in the middle back T6-7. It is my opinion This entire report is your opinion. that she has reached maximum medical/chiropractic improvement and has been advised to have supportative This is spelled wrong and grammar counts…It’s bad enough you look the fool…Don’t look illiterate as well!!! treatment on a once per month basis over the following year at an average cost of $110 per visit.
The tenderness that she continues to have in the neck and middle back are result of torn muscle and ligament tissue How do you know this? You have no MRI’s, CT scans or any other tests. This is a very poor guess that is unsubstantiated by anything that you have done according to your report. which heals with fibrotic scar tissue which is permanent and predisposes This is all guess work and crystal ball stuff, all of which is inadmissible in court. the patient to future pain and spinal degeneration. She will suffer from occasional flare ups of pain in the injured areas throughout her life. Again, more inadmissible guesses that will make you look like a fool with the lawyer.
If you want to have a huge PI practice, you had better do a better job than this. This doctor also elicited serious head trauma findings, yet didn’t order brain scans or specialist referrals. It is only a matter of time before a doctor like this has a patient that has serious seqeulae and possibly dies…Then the doctor will lose his license.
The problem is that the doctor can find something else to do. The patient can’t…forever. This is serious stuff and we are entrusted with lives. Therefore, when I say we all need to be the best through clinical excellence, it far transcends building a practice. It is about saving lives. This doctor will get lucky for a while, then the wrong patient will be in his world…As for the lawyer, this doctor is forever a “one and done!”
