Preamble: Many of the issues I bring to you are very small, yet each issue is just that, an issue. If you take care of the small issues, then you will be able to build and more importantly, focus on the bigger issues...a larger practice and more family time.

"The Simple Stuff That Devastates Your Practice"

Do you want to get angry? How about your claim not being paid or having a retrospective review because you did notput the date on the evaluation or SOAP note? This should make you angry…at yourself. This is one of the most common elements of failing an audit because if there is no date on a document, it doesn't exist, as you cannot certify when the service was done. The date when the report was done needs to be on the document and if the date of service is different than the report, that must be clearly delineated.


Is your name printed at the end of the document so the carrier knows who signed off on the service? I have seen your signatures…No one is deciphering most of them, especially mine. Many states consider an original signature a requirement for verification purposes that a service was done and if missing, can legally not pay you for a service. Therefore, unsigned documents or ones with a signature stamp should never leave your office.


In addition, I promise you that your staff is abusing your signature stamp if you have one. My recommendation, in the strongest of terms, is to take and burn it immediately. If you have to sign too many forms…too bad. Make your signature as an ”X” if you need to, but make your mark and do not give anyone else the power over your practice.


In addition, more stupid stuff…Is the patient’s name on every page of a multiple page document? In today’s world of technology, everything is scanned and separated. Therefore, many documents get scattered and leave the examiner the chore of figuring out who goes with what report. This is a very common scenario because many do not have access to what they scan. Put the patients name on every page.
