It makes perfect sense to create this level of transparency and establish a legal precedent on each case from the first visit.
In your E&M reports for ONLY personal injury cases, consider starting with this statement
" Mrs. Jones was in an automobile crash on January 12, 2021 and but for this causally related incident, would not have the following clinical findings."
This statement removes any ambiguity, issue or perception regarding causality. It is a powerful statement. This permanent change should also be shared with your lawyers to tell them you understands their needs and make you more valuable to them.
