Thank you Dr. Clay Wickiser from South Carolina for the images. This patient is exhibiting myelopathic clinical findings of right leg weakness and diminished patellar reflexes. Although there is not clear compression, there are hints in the images below. Coupled with the clinical findings this patient is off to the surgeon with a diagnosis of Myelomalacia for the surgeon to consider and co-manage.
