“Head Trauma Triage”

I received this one minute ago…

 
Have a question.  I have a new patient that was rear ended at high speed and actually broke her foot which was on the brake pedal while stopped.  The hospital did x-rays and CT of her head.  She complains of being in a fog all the time.  I know it is from the brain.  What MRI study do I order? 

The answer: I want the patient in a neurosurgeon’s office immediately and let them decide the best course of diagnostics. There are 2 reasons 1) The patient, although rare, but not uncommon might have a “pin hole” in the brain vessels and could have a “wake and die syndrome,” and you need to prevent that!!! 2) You also need to protect yourself from any uncommon co-morbidities.

 
This does not mean you release the patient; you are just sending them for a consult with the full expectation of getting the patient back into your office.  Make sure that is known to both the surgeon and the patient.
