When looking at national trends, I get to see what carriers are doing and stay ahead of their bullcrap reasons for denials. The newest trend in late 2018 and 2019 for denials are surrounding causality. The carriers are stating that the visits after the E&M visit are not causally related to the accident although, in your E&M report, you clearly stated the reason for future encounters.

 

Therefore in EVERY SUBSEQUENT SOAP note, you need to add the following macro:

“The reason for this visit is for injuries sustained as a result of the motor vehicle accident dated xx/xx/xxxx as described in the initial evaluation.”

 

Your current EHR should handle this and if you are still on paper (you poor bastard), add this is a checkbox and fill in the date every time.
