“End Results of Poor Documentation”
"Bad things happen to good people” Mark Studin 2018
The above image is a significantly redacted finding by a state licensure board on a very good Dr. As you can see above this doctor was fined a total of $54,304.00 and lost his license for five years, after which he is not auto reinstated by, he has to reapply for his license and take whatever actions the board deems appropriate to reinstate it (with no guarantee to be license again).
This is just a sample of a long list of licensure actions that I come across on a regular basis and why I am so tough on compliance reviews and persistent in my recommendations. More so, this is why we have created a transcription service with oversight so that you have an extra set of eyes reviewing your documentation to ensure that you do not miss even the basics. A pattern of minor errors that persists in many patients can be construed as a RICO violation because you are using a paper instrument to defraud a financial institution systematically.
I share this with you not to create FEAR as I do not choose to function in that environment. However, I share that with you so that you understand the reality of my words and you are highly motivated to take my recommendations. Also, Wall Street has also weighed in and realized that our level of oversight in documentation is as close to a “guarantee” to increase their return on investment which equates to you getting paid at a higher level if you follow the recommendation (and mine).
Please note that the most devastating determination from that state boards report above is five years of no practice. To learn more about the oversight program with evaluations and SOAP notes that for youCLICK HERE.
