I received this from Dr. Rhett Bogacz from Florida; It is EXCELLENT. 

Past Medical History

Ms. Jones has a history of a previous automobile accident in December 2005 where she injured her neck/back.  She was treated for her injuries with her last treatment in 2007.  She also has a history of stomach ulcer with her last treatment 2016. Mrs. Jones has a history of Fibromyalgia, anxiety and insomnia for which she has been deemed disabled.  She is currently under the care of Dr. Primary for these conditions. Ms. Jones was also treated in my office for neck, upper back, and shoulder pain on 3/27/2017 and was released from active treatment on 5/11/2017.  

This past history is complete and “closes the door” on all past treatment, a Colossus and court requirement to help establish current causality. I strongly suggest that you follow this format in your reports, both in E&M and Narrative formats.
