Preamble: many of these issues I bring are small, yet each issue is just that… an issue. If you take care of the small issues, then you will be able to build and focus on the larger issues… a larger, more profitable practice along with more family time. 

“The Proper Use of the Patient’s Name”

I am seeing a pattern of mis-use of the patient’s name in both evaluation reports and narrative reports. It is NOT acceptable to use the patient’s first name. 

“As a result of the injuries, Delores cannot lift her child for more than 5 minutes…”

Delores??? Is she your girlfriend? Are you having sex with her? Why are using such a familiar name? It leaves too many issues open that will hurt you. ALWAYS in the written forum use the patients surname. 

The proper use: 

“As a result of the injuries, Mrs. Smith cannot lift her child for more than 5 minutes…”

This is non-negotiable!!!! As the improper use can hurt you in too many ways.
