"Home Care / Lifestyle Recommendation"
In the past, I have recommended to you that "home care" recommendations are an integral part of the evaluation process and should be documented as such on each evaluation and re-evaluation. Based upon feedback from the courts and lawyers, these recommendations need to be added to the narrative to certify permanencies and the basis for lifelong care.
In addition, "lifestyle change" recommendations fall under this same category because if somebody has a permanent problem, these changes go hand-in-hand. As an example, if I have a permanent low back problem, I might be precluded from playing any sport which increases my axial loading such as jumping up and down in basketball. I might also be precluded from lifting anything greater than 15 pounds. Therefore, in your evaluations and re-evaluations, be sure to have home care and lifestyle recommendation sections that are reflective of your patient's current status. In your last re-evaluation prior to the narrative, this should be considered a long-term recommendation and documented as such so that your narrative reflects your evaluation.
These recommendations should be listed in the narrative just before the conclusion and cryptically repeated in the conclusion. I will be doing a full consultation on sample language to be added into the narrative within the next few days. Please make sure that you add this to your evaluations.
