RE: Final Narrative Report-William Patient

If this is what you use in the heading of your narrative reports and the patient has a subsequent exacerbation, you just lied on your report. If you testify to this in court and then produce another record due to an exacerbation, you just perjured yourself on testimony and that is a felony. 


NEVER… EVER… NEVER… NOW or EVER… NEVER

 
Use any time relationship to a narrative report in a header except the following:

 
Date of Loss: 06/07/2016
Date of First Office Visit: 06/08/2016

A narrative is simply another report and carries no descriptive heading. Do not use the word narrative, report. Nothing, zip, nada, goonish, …nothing. Therefore, the opening AFTER the business format heading with the lawyer’s name, firm name and address (you are a real business, so present yourself as such) should look like this:
