Preamble: Many of the issues I bring to you are very small, yet each issue is just that, an issue. If you take care of the small issues, then you will be able to build and more importantly, focus on the bigger issues...a larger practice and more family time.

"Thecal Sac”

The thecal sac is the covering of the spinal cord and spinal nerves. It is comprised of the dural and arachnoid layers of which the arachnoid is both innervated with A-B-C pain fibers and has a vascular supply. When a disc herniates or bulges, the first structure effected is the thecal sac and independent of the cord or roots, because it is comprised of pain fibers, there can be localized pain from simply contact with the thecal sac. There can be NO radiation of pain from thecal sac involvement. 

The picture below shows the thecal sac denoting it as the dura. Although it is listed in some literature as the dural sac, it is universally called the thecal sac by almost 100% of radiologists and specialists who treat patients involving this structure.
